Overview

• Outpatient Physical Therapist

• Contract length: 13 weeks
• Schedule: Monday-Friday
• Shift options: 8-hour shifts or 4x10-hour shifts, to be determined with the manager

Required Qualifications

• Valid Physical Therapist license in Massachusetts
• BLS issued by American Heart Association or American Red Cross
• License/certificate verification
• Flu vaccine
• MMR documentation or proof of immunity
• TB test documentation
• Health statement signed by an MD/DO/NP/PA
• Criminal background check
• Massachusetts CORI
• Resume including past 7 years of work experience
• 2 references within 4 years of submission, with at least 1 supervisor or above in the skillset
• Skills checklist completed within 1 year of submission
• Education verification
• SSN trace
• National sex offender check
• Exclusion checks

Onboarding / Compliance

• MGB MVH Housing Agreement, required only if moving into shared housing
• Confirmation of Assignment/POI form
