Physical exam: Within 1 year of start and updated annually; signed by an MD, DO, NP, or PA (not a chiropractor)
Tuberculosis: Negative QuantiFERON/T-SPOT or 2-step PPD within 1 year of start, updated annually; positive results require a chest X-ray and TB questionnaire
Vaccinations and titers: Measles, mumps, rubella, hepatitis B, varicella, and Tdap requirements (titer or vaccine series, or signed declination where allowed)
COVID: Proof of vaccination or signed declination
Influenza (2026-27): Current seasonal flu vaccine or facility-approved exemption required; declination forms not accepted. Onsite vaccination available.
Respirator fit test: Within 12 months of start, updated annually, using an approved 3M mask (1860S, 1860R, or 1870/1870+) or universal sizing; onsite testing available
Color vision: Ishihara exam completed before start; must be done in person, not online
Drug screen: Within 30 days of start, updated annually
Competency exams: Required within 1 year of start, updated annually, with a minimum score of 80% on core and OSHA exams
Orientation: Facility modules must be completed within the first two weeks
