Physical Therapist (PT)

Location: Haverhill, Massachusetts

Contract Length: 13 weeks

Required Qualifications

• Valid Physical Therapist license in Massachusetts
• Physical Therapist (1 year of experience)
• Physical Therapist experience within the last year
• Experience in a Long Term Care or Rehabilitation setting

Role Summary

• Screens, evaluates, and rehabilitates patients with physical functioning disorders and/or wound care
• Develops and adjusts plans of care using standardized assessments and evidence-based practice
• Performs rehabilitation procedures including manual techniques, ambulation, therapeutic exercises, modalities, supportive and assistive devices, and wound care
• Communicates patient progress, barriers, and treatment plans with the supervisor and other health team members
• Documents required information in the electronic documentation system and maintains patient records
• Records treatment minutes and provides discharge summaries
• Participates in required meetings, trainings, in-services, and conferences
• Obtains necessary DME or supplies
• May supervise and support physical therapist assistants, rehabilitation aides, and/or students

Practice Standards

• Must adhere to APTA and state-required practice acts, code of conduct, and ethics
