Travel/Contract assignment in Portland, Oregon
13-week contract
Day shift: 5x8 hours, 8:00–16:30
40 hours per week

Required Qualifications

Valid RN license in Oregon
Home Health (1 year of experience)
Home Health experience within the last year
One completed travel assignment
BLS issued by American Heart Association or American Red Cross
Associate of Science in Nursing / Associate Degree in Nursing
Driver's License
Experience with OASIS
Experience with Homecare Homebase

Notes

Proof of current auto insurance is required for candidates whose job responsibility includes driving, and is required for all Home Health positions.
An NPDB self-query is required for candidates placed in Oregon Home Health only.
Candidates must have full 7 years of work history in the application with no gaps.
Seven years of employment history must be verified.
References must be completed within 2 years of start.
Fit test required for all candidates; N95 accepted.
TB Questionnaire is required annually.
Physical exam must be within the last 12 months.
Drug screen must be within the last 12 months of start.
Background check and related screenings must be completed within 90 days of start.
