Case Manager

Role Overview

Travel/Contract RN role in Oakland, California.
Facility-based assignment at Alameda Highland Hospital.
13-week contract.
Day shift: 5x8 hours, 8:00–16:30.
40 hours per week.
Auto offer.
Every other weekend required.
Float scope: all inpatient units.

Required Qualifications

Valid RN license in California.
Case Management experience within the last year.
1 year of Case Management experience.
BLS issued by American Heart Association.
Associate of Science in Nursing / Associate Degree in Nursing.
Experience making referrals to lower level of care facilities.
Ambulatory Care experience.
Home Health experience.
Hospice experience.
Skilled Nursing experience.
Long Term Acute Care experience.
Experience with Epic.
