Step-Down RN

• Location: Auburn, New York
• Employment type: Travel/Contract
• Contract length: 13 weeks
• Shift: Night, 19:00 to 7:30 (3x12-hour shifts)
• Hours: 12 hours per day, 36 hours per week guaranteed
• Scheduling: Every other weekend and every other holiday; no blocked scheduling
• Local candidates: Accepted

Unit and Patient Population

• Ratio: 1:4-5
• Diagnoses: Congestive heart failure (CHF), s/p heart catheterization, s/p vascular procedures, respiratory failure (BiPAP/CPAP/Vapotherm), cardiac dysrhythmias
• Unit procedures and expectations: Post-heart cath care, TR bands, post-vascular procedure monitoring, frequent vital signs and neurovascular checks

Required Qualifications

• Valid RN license in New York
• Stepdown experience: minimum 2 years, with experience within the last year
• ACLS (Advanced Cardiac Life Support) issued by the American Heart Association
• BLS issued by the American Heart Association
• NIHSS (NIH Stroke Scale)
• Associate of Science in Nursing (ASN) / Associate Degree in Nursing (ADN)
• Experience working with cardiac patients
• Experience working with neuro patients
• Dysrhythmia interpretation
• Dysrhythmia management
• 12-lead EKG/ECG
• IV insertion skills
• Experience with the care and management of invasive lines
• Blood product administration
• Experience with conscious or moderate sedation
• Experience with cardiac drips

Background and Screening

• Background check completed within 1 year of start, covering all locations lived, worked, or in school for the past 7 years, updated annually. Instant state searches are not acceptable as a placeholder for pending counties; results must be complete before working.
• EPLS/GSA/SAM and HHS/OIG checks, required before facility start
• NYS Medicaid Exclusion List check
• NYS Nurse Aide Registry (required by facility)
• 2 professional references within 3 years of hire
• Automated dispensing cabinet security training (required for access to medication dispensing)

Health Requirements

• Full physical with review of systems within 12 months of start, signed by a licensed provider, stating the employee can perform job duties. Physician statement or physical form is not acceptable.
• Fit test within 12 months of start, updated annually, using an approved mask model
• TB screening: PPD, T-Spot, or QuantiFERON within 1 year of start, or chest X-ray if TB-positive (within 5 years), plus annual TB questionnaire
• TDAP within the last 10 years, or declination
• Measles, mumps, and rubella immunity: 2 MMR vaccines or serologic titers (presumptive immunity options available). Measles and rubella immunity are required by NYS regulation; mumps immunity is recommended and status must be provided.
• Hepatitis B series, titer, or signed declination
• Varicella: 2 vaccines, positive titer, physician-documented history of disease, or negative titer with signed declination
• 9-panel drug screen within 30 days of start (instant/rapid accepted); not required annually
• Flu vaccine or declination required for clearance during flu season (9/1-5/1); candidates declining must wear a mask during flu season; documentation due by 11/30 for working clinicians
• COVID vaccine records or declination

Facility Documents

• Code of conduct, patient abuse reporting, and confidentiality/non-disclosure agreement (review and sign)
• Orientation packet with self-study orientation quiz and signed acknowledgments
