LVN / LPN Home Health – Portland, Oregon
Contract Length: 13 weeks Start Dates: 11/16 and 11/23 Employment Type: Travel/Contract (Offer position; you will not connect with the manager before signing. The facility has 48 hours after signature to review the clinician.)
Setting and Schedule
- Setting: Skilled Home Health
- Shift: Day shifts, 5 x 8 hours (8:00–16:30), 40 hours per week
- Patient Ratio: 1:1
- Productivity: Approximately 30 points per week, up to 6 patients per day
- Weekends: Weekend support may be required
- Call: None
- Service Area: Usually local, but may occasionally extend up to about 60 miles, or 60–90 minutes of total driving per day. The facility groups patient visits and minimizes driving where possible.
Requirements
- Valid LPN / LVN license in Oregon
- Active Oregon and Washington license, posted before the start date (temporary licenses not accepted)
- LPN / LVN experience within the last year (minimum 1 year of experience)
- Home health experience (at least 1 year of skilled home health experience)
- One completed travel assignment
- Experience with Home Care Home Base (HCHB) documentation system
- Experience with OASIS
- BLS certification issued by American Heart Association or American Red Cross (AHA, ARC, or military accepted)
- Valid driver's license
- Reliable personal transportation
- Proof of current auto insurance (required for all home health positions)
Technology
- Home Care Home Base (HCHB) documentation system
- Remote client for home health on a laptop
- Rover phone application
Onboarding and Compliance
- Background check within 90 days of start, updated annually, including an NPDB self-query for Oregon home health placements
- Exclusion list checks (EPLS/GSA/SAM and HHS/OIG), with HHS/OIG run monthly
- Education verification for highest degree related to profession
- Work history verification covering 7 years
- Drug screen (9-panel) within 12 months of start
- Physical exam statement within the last 12 months
- TB questionnaire annually, plus TB test within 1 year of start (or chest X-ray if PPD is positive)
- Measles, mumps, rubella, hepatitis B, and varicella immunity documentation (vaccines, titer, or declination)
- Flu vaccination or facility declination form, and COVID vaccination or declination
- N95 fit test
- Acceptable use policy, safety manual acknowledgment, and agency attestation completed before start
- Two professional references within 2 years of start
- Competency exam and skills checklist (annual)
Time Off
- Up to 3 days of RTO may be automatically approved
- The client can approve 1 holiday: Thanksgiving, Christmas, or New Year's
Facility Information
- Attire: Business casual, no jeans. Scrubs are permitted but not required, with no specific color.
