Schedule and Assignment Details

• Float between units: Yes
• Float between all three locations in the system, including the facility's sites in the same system
• Float scope: Shifts may rotate. The employer will try to provide a consistent pattern when possible.
• Alternating weekends required
• On-call: Not required
• Block schedules: Not required
• Patient ratio: 1:16
• 48-hour weeks: Not approved
• Scrub color: Any color (OR, Critical Care, ED, and Maternal Child areas have scrubs provided)
• Parking: No fee
• Charting: EMR system (EMR training may be required)
• Float policy acknowledgement required. The traveler may be required to float to another facility in the system within a reasonable distance.

Requirements

• Valid Licensed Clinical Social Worker (LCSW) license in Oregon
• Licensed Clinical Social Worker experience of at least 1 year, including experience within the last year

Onboarding Requirements

• Identity and employment: Government-issued photo ID, Social Security card (for I-9), and E-Verify attestation
• Background check: Completed within 90 days of start, covering all locations lived or worked in the past 7 years. Must be updated annually. Includes education verification (third-party).
• Additional searches: Federal exclusion and sanctions checks (within 30 days of start, updated monthly), and an Oregon abuse/neglect search, which applies only to placements at the system's child center
• Drug screen: 9-panel drug screen within 12 months of start. Instant results are acceptable.
• Physical exam: Within 1 year of start, signed by an MD, NP, PA, DO, or APRN. Must indicate freedom from infectious disease and ability to work with or without accommodation.
• TB screening: Negative IGRA, QuantiFERON Gold, or 2 PPDs within 12 months of start
• Immunizations: Tdap within 10 years (declination accepted). MMR (measles, mumps, rubella), varicella, and hepatitis B proof or signed declinations are required.
• Flu vaccine: Required during flu season or a signed declination
• COVID: Most recent vaccination record preferred. Declinations are not required.
• Respirator clearance: Medical clearance for respirator use, renewed annually
• Fit testing: Mask fit test required. Any mask type is currently accepted.
• Badge photo: Passport-style with a neutral background
• References: At least 2, covering 1 year of employment within the last 3 years in a comparable specialty or setting
• Employment application: Must cover at least the last 5 years, with month/year dates and explanations for any gaps
• Training: Annual facility modules (about 3.5 hours) required for all candidates
