Home Health Physical Therapist (Days)

• Location: Fort Dodge, Iowa
• Employment type: Permanent, full-time
• Schedule: Monday through Friday, 8:00 am to 4:30 pm (8 hours per day)
• Setting: Home health; visits are made to patients' homes

Role Overview

The Physical Therapist uses clinical expertise, physical assessment, teaching, and skilled therapy interventions to deliver high-quality care in the patient's place of residence. The PT develops or contributes to the home health plan of care under physician direction and coordinates interdisciplinary involvement.

Key Responsibilities

• Make home visits and perform initial and comprehensive assessments under agency policies
• Evaluate function, treat to relieve pain and restore function, and initiate diagnostic, preventive, and rehabilitative procedures
• Develop, update, and communicate plans of care and discharge plans, including measurable goals, with the patient, caregivers, and physicians
• Teach patients and family about treatment, medications, equipment safety, and emergency preparedness
• Receive and document verbal orders, and obtain physician orders before starting care
• Supervise home health aides and physical therapy assistants, and document that supervision
• Maintain accurate, timely clinical documentation and time/travel entries, and maintain competence in OASIS data collection and submission
• Identify risks for hospitalization or emergency department use and intervene proactively

Required Qualifications

• Valid Physical Therapist license in Iowa
• Physical Therapist experience (1 year of experience)
• Physical Therapist experience within the last year
