Registered Nurse - Hospice

Overview

• Location: Woodward, Oklahoma
• Employment type: Travel/Contract
• Contract length: 13 weeks
• Position: RN
• Shift: Day, 5 x 8 hours (8:00 to 17:00), Monday to Friday
• Hours: 8 per day, 40 per week scheduled
• Guaranteed hours: 20 per week with a 40-hour schedule
• Average caseload: 5 to 7 patients per day
• Case types: Geriatric

Required Qualifications

• Valid RN license in Oklahoma
• Hospice experience: 1 year minimum, with hospice experience within the last year
• Home Health experience
• Experience with OASIS
• Experience with Homecare Homebase
• One completed travel assignment
• Associate of Science in Nursing (ASN) / Associate Degree in Nursing (ADN)
• BLS certification issued by American Heart Association or American Red Cross

Software Familiarity (not a credential)

• HCHB EMR experience requested

Other Requirements

• On-call: Required; to be discussed during interview
• Territory: Up to a 60-mile radius from the hub/office
• Special requirement: Traveler must be flexible with territory
• Local candidates: Accepted
• First-time travelers: Not accepted
• Dress code: Scrubs in any color; no jeans

Background and Health Requirements

• Background check: Completed within 1 year of start, covering all locations lived, worked, or attended school, and updated annually. Search includes SSN trace, counties, nationwide, sex offender registry, and MVR.
• Exclusion screenings: EPLS/GSA/SAM and HHS/OIG, required before facility start
• Physical exam: Physical statement within the last 12 months of start; not required annually
• TB: Negative PPD, QuantiFERON, or TSpot within 1 year of start, or chest X-ray within 5 years of start for a positive PPD (verbal positive confirmation acceptable); not required annually. TB questionnaire required for a positive TB result and annually.
• Immunities (Rubeola, Mumps, Rubella, Varicella): Two vaccines or positive titer. Negative or equivocal titers require declination, two vaccines, or one booster. Candidates born before 1957 are exempt.
• Hepatitis B: Series, positive titer, or declination. Negative or equivocal titers require declination or one booster.
• Drug screen: 9-panel drug screen (instant acceptable) within the last 12 months of start; not required annually. Testing is conducted only for cause, but completion is required.
• Flu vaccine: Vaccine or declination during flu season (10/1 to 3/31), 2026-27

Onboarding Documents and Other Steps

• Badge photo: Required within 72 hours of signing (time sensitive). Must be a clear, passport-style photo on a white solid background.
• Professional references: Two within 3 years of hire
• Orientation safety manual acknowledgement: Signed; updated annually
• Welcome videos: Must be watched; nothing to turn in
