Job Summary (List Format): 
 
- Provide comprehensive rehabilitative services to patients in both rural health clinic and inpatient settings 
- Perform skilled physical therapy evaluations 
- Develop and implement individualized treatment plans 
- Collaborate with interdisciplinary healthcare team members to optimize patient recovery and independence 
- Deliver high-quality, evidence-based care 
- Educate patients and families on therapeutic techniques, use of equipment, and home-based exercise programs 
- Ensure compliance with professional standards and facility policies 
- Maintain required documentation and patient records 
- Hold a current Wisconsin Physical Therapist license 
- Maintain current BLS certification 
- Work flexible hours during clinic times (6:30 am to 5:15 pm), based on patient and clinic needs
