Job Summary: Registered Nurse (RN) – Hospice 
 
- Provide skilled nursing care to hospice patients in a home health setting 
- Deliver patient and family education and support regarding end-of-life care 
- Manage patient triage, goal setting, and symptom management 
- Utilize Home Care Home Base (HCHB) software for documentation and care coordination 
- Collaborate with interdisciplinary hospice team to ensure optimal patient outcomes 
- Work Monday-Friday, day shift (8:00 AM – 4:30 PM) on a 13-week contract 
- Ensure compliance with all regulatory requirements and best practices 
- Maintain active WA RN License (or Compact License) and BLS certification 
- Apply minimum 1 year of hospice/home health experience in all aspects of patient care
