Job Title: RN Care Manager (Home Health)
Location: Sonora, CA 95370

Weekly Hours: 40 hours
Daily Hours: 8 hours

Job Description:

Plans, organizes, and directs home health services to meet patients’ needs in their homes. Manages an assigned patient caseload efficiently and effectively. Provides independent patient care while collaborating with other healthcare disciplines to achieve patient care goals. Serves as a resource for patients, family members, and other care team members.

Must-Have Requirements: Active RN license in the state of practice – Required Current CPR/BLS certification from an approved vendor – Required Valid driver's license – Required Current automobile insurance and a reliable vehicle – Required EPIC experience – Required Day shift availability – Required Pay Bifurcation: Taxable: $1848.00 Non-Taxable: $1190.00 Total Gross: $3038.00/Week
