DescriptionKlarus Home Health / Jet Health, Inc is currently seeking a PRN / Per Diem Licensed Clinical Social Worker to join our team to serve our patients currently enrolled in our home health services in Granbury, TX and surrounding areas. Position could see 3-4 visits per month The successful candidate will provide coordinated care in the home to patients of all age groups, performing psychosocial assessments, analysis, counseling, and referrals to meet the needs of the patient, client, and family.
