13-Week Contract | Day ShiftWe’re seeking an experienced Travel Physical Therapist (PT) for a 13-week home health–focused outpatient assignment. This role is best suited for a seasoned traveler who is comfortable managing a mobile caseload and documenting in a home-based care environment.<h3>Assignment Details</h3><ul><li>Shift: Days | Monday–Friday (Missed visits may be made up on weekends)</li><li>Length: 13 weeks</li><li>Guaranteed Hours (GT): 36 hours/week</li><li>Setting: Outpatient with approximately 20% Assisted Living visits</li><li>Patient Load: 6–7 patients per day</li><li>Charting: WellSky (HomeCare/HomeBase experience required)</li><li>Travel Radius: Within 50 miles of the facility</li><li>Mileage: Included in bill rate</li><li>Expected Weekly Mileage: 50–75 miles (may vary)</li><li>Facility Work Week: Sunday–Saturday</li><li>Meetings: Daily stand-up meetings at 8:30 AM (Mon–Fri)</li><li>Dress Code: Clean, professional scrubs with closed-toe shoes</li></ul><h3>What You’ll Do</h3><ul><li>Perform patient evaluations and develop individualized plans of care</li><li>Provide evidence-based PT interventions in outpatient and assisted living settings</li><li>Manage visit scheduling and complete timely, accurate documentation</li><li>Participate in daily interdisciplinary stand-up meetings</li><li>Educate patients and caregivers on mobility, safety, and recovery</li></ul><h3>Requirements</h3><ul><li>Active Physical Therapist license</li><li>Minimum 2 years of recent licensed PT experience (required)</li><li>At least 1 recently completed travel assignment (required)</li><li>Home health experience with HomeCare/HomeBase (required)</li><li>BLS &amp; First Aid certifications (required)</li><li>Strong organization, communication, and time-management skills</li><li>No new graduates</li></ul><h3>Additional Requirements</h3><ul><li>COVID-19 vaccination and boosters required (Medical/Religious exemptions reviewed case-by-case; approved exemptions require weekly testing provided by the facility)</li><li>Housing must be secured prior to submission</li></ul>Apply today if you’re an experienced travel PT seeking a dynamic home-health-focused assignment with consistent weekday hours.
