How you’ll make an impact in this role

• Coordinate interdisciplinary rehabilitation care in close partnership with PT, OT, and SLP specialists to ensure unified, progress-driven treatment plans.
• Monitor and communicate critical health changes in real time, delivering accurate clinical assessments to physicians to maintain safety during intensive therapy.
• Execute direct nursing interventions and physician orders with precision, supporting physical recovery and helping patients rebuild daily functional independence.
• Drive comprehensive discharge planning alongside the care team, empowering patients and families with the tools and education needed for a safe transition home.

What minimum requirements you’ll need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• One or more of the following required:
  • Licensed Registered Nurse credentialed from the Tennessee Board of Nursing OR current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of
  hire.
