Care Coordinator - RN Full Time Professional Watseka, IL, US We are currently seeking a compassionate, and enthusiastic Care Coordinator to join our friendly team! Job Title: Care Coordinator - RN Department: Satellite Clinic Status: Full Time, 40 hours per week Schedule: 8:00 AM - 4:30 PM Summary: The ideal candidate must have a valid Illinois RN/LPN license, hold a current BLS certification, have great communication and interpersonal skills, computer skills, organized and ability to build relationships. The Care Coordinator will serve as an extension of the primary care team in coaching, implementation of interdisciplinary care plan and care coordination for high risk patients with multiple comorbid conditions. This approach is an enhanced evidence based skill set using the integrated chronic care disease management model. The Care Coordinator will implement clinical interventions such as chronic care management services and serve as a liaison for coordination between patients and members of the care team. Focus is on coordination of patient care through a continuum and facilitates the achievement of optimal outcomes in relation to clinical care, quality and cost effectiveness. The position will be a key integrator and coordinator to help patients bridge the gaps that occur in care transitions. Required Education and Experience * Graduate of an accredited school of nursing. * Previous experience in caring for chronic disease patients required. * Minimum of 5 years experience in healthcare setting, preferably primary care. * Demonstrates evidence of essential leadership, communication, education, collaborations and counseling skills. * Proficient in communication technologies. * Previous experience with health IT systems and data reports preferred. * Previous experience with mobilizing community resources and navigating through the healthcare continuum is preferred. * Knowledge and sensitivity to communication strategies, accessibility and ability to identify appropriate communication strategies when required. The hospital operates rural health clinics in Watseka, IL, Gilman, IL, Milford, IL, St. Anne, IL and Kentland, IN. The hospital also operates a skilled nursing facility, Iroquois Resident Home, which is attached to the hospital property.
