Registered Nurse - Multiple Openings (Primary Care)


Cascadia Health has various opportunities for RNs. RNs will work both in our residential programs as well as our health center programs. Shifts vary by program. Residential programs have day shifts, and some swing and overnight in our secure residential. Both Swing and overnight shifts are provided with a shift pay differential in addition to the hourly wage when clocking in. Float shifts and Per Diem (on-call) are given a $1/hr pay differential. Most health center programs have day shifts. Programs vary but residential programs include Choice Model, Psychiatric Review Board (PSRB), and Secure facilities, Supported Housing, and Respite. Health Center programs include Child and Family, Primary Care, Older Adults, SUD services and more. The Registered Nurse (RN) coordinates the provisions of treatment for the shift and implements medical interventions. The RN assumes responsibility for treatment decisions in keeping with RN licensure provisions and renders care prescribed by physicians and nurse practitioners. All incumbents must understand, appreciate and respect the diversity and cultural differences within our Cascadia community. It is expected that there is an awareness and sensitivity to the perspectives and perceptions to those we serve, as well as our colleagues. As Cascadia is Trauma informed, the incumbent must support a Trauma Informed Care Environment. With the high prevalence of trauma among individuals receiving behavioral health services, it is required that the incumbent understand the effects of trauma on health, coping, and other aspects of the lives of those we serve. Click here to learn about the benefits we offer and more Responsibilities This position description is not intended to be an all-inclusive list of responsibilities or an all inclusive job description. * Provide quality nursing care, including all medical interventions and appropriate documentation. * Formulate Nursing Care Plans for all medical/physical conditions. * Administer PRN medications based on presenting systems and proper assessment to determine the need for the PRN. * Participate in planning, implementing, evaluating and documenting direct resident care activities based on sound psychiatric for secure residential sites * Act as liaison with medical staff, pharmacy and laboratory services when appropriate to assure continuity of quality resident care. * Place strong emphasis on community reintegration, utilizing a comprehensive rehabilitation program that includes therapy and skills-building groups, dual-diagnosis treatment, nutrition and exercise, a vocational training program and staff-escorted community outings. * Administer psychotropic medications, interpret resident response to medication and take appropriate action based on current psycho-pharmacy standards. * Collaborate and participate as a team member to establish medical treatment plans and resident care. * Interact appropriately with residents, visitors and other community members to support the treatment process, and advise on resident rights to treatment, refusal of treatment and involuntary treatment procedures as provided by applicable regulations. * Document and maintain electronic health record (EHR) and ancillary records in a manner that assures compliance with all agency policies, program procedures and local, state, and federal regulations. Maintain accurate and up-to-date documentation as required, associated with billing, assessment, treatment, client's progress and other activities. * Demonstrate knowledge of the kinds of resident medical conditions most commonly encountered and understand the physiological basis for the nursing care delivered. * Inform residents about their medications and medical treatments. * Assist in crisis situations as appropriate, following emergency protocols/procedures and coordinating with the treatment team with interventions which support a trauma informed environment for residents and staff. * Adhere to mandatory abuse reporting laws and HIPAA requirements. * Call the Licensed Medical Practitioner (LMP) with laboratory results. * Coordinate with oncoming shift regarding resident concerns, building/program needs, and any issues relevant to ensure the smooth operation of the program. * Responsible for administration of seclusion and restraint techniques as defined in non-violent physical intervention training, agency Policies and Procedures, as well as applicable Oregon Administrative Rules. * Be a part of a rotation to remain working on-site for up to eight (8) additional hours in the event that the following shift has inadequate staffing for residential programs. * Complete annual employee training requirements on a timely basis, attend trainings and staff required meetings. * Abide by HIPPA regulations and mandatory abuse reporting and other federal, state, and program compliance, regulations and rules. * Perform other duties as assigned Qualifications * An active, unencumbered Oregon Registered Nurse License and CPR/First Aid Certification * Some programs will require the RN be eligible to be QMHA or QMHP certified: Must either have a BSN or 1 yr. of mental health related work experience in addition to an ADN/ASN to meet this eligibility. * A minimum of two years of clinical experience providing inpatient or outpatient services is preferred. * Knowledge of the kinds of medical conditions most commonly encountered by people with a severe and persistent mental illness and an understanding of the physiological basis for the nursing care delivered. * Motivational interviewing and ability to maintain a Trauma Informed Care environment * Sufficient computer skills to complete documentation in an electronic medical records system in a timely manner * CPR/First Aid certifications are required and must remain current. * Driving may be required for some programs, so it is preferred to Possess a valid drivers license, three year driving history; clean driving record, ability to pass Driver Motor Vehicle (DMV) check and complete the Cascadia online Defensive Driving course. Physical Demands * Ability to perform CPR, First Aid, and properly use AED equipment. * A typical workday may consist of sitting, utilizing computers and other office equipment. * Some program may require the RN to transport clients out in the community using an agency vehicle * The Registered Nurse must have the ability to assist the team in providing safety to all clients and staff in potentially volatile situations. * Occasionally stooping, lifting (20 to 50 lbs.), climbing (stairs), crouching, reaching, and running * Regular sitting, Standing, walking, and Keyboarding * Constant visual and auditory Cascadia is an Equal Opportunity employer.
