Core values: Commitment Advocacy Respect Excellence About the Position: The goal of the home health Registered Nurse (RN) is to return the patient to the highest practicable level of independence and well-being within the community with self-management of disease and other identified processes. When the highest practicable level of well-being is not independence, the goal is to prepare the patient and family/caregivers for a continuum of care within the home and/or within other community-based health care systems. Pay: $75,000-100,000 * Optimal Pays Per Visit Qualifications Qualifications: * A graduate of an accredited, approved school of professional nursing. Must have home health experience. * Active license in the State of Colorado in good standing. * Must be able to communicate in the language of the client and document client related-activity as appropriate * Current driver's license * At least one (1) year of experience as a Registered Nurse. Home Health experience is a plus. * Familiarity with OASIS preferred
