Summary

Job Summary:

Lifebridge Health is seeking a dedicated Nurse Navigator to lead and coordinate the Cancer Survivorship and Integrative Medicine program across our network. This role will focus on providing comprehensive support to cancer survivors, guiding them from diagnosis through active treatment and into survivorship. The Nurse Navigator will also develop, implement, and manage resources, workshops, and support systems that promote wellness, symptom management, and quality of life for survivors. Working closely with the System Medical Director, the Nurse Navigator will help grow and enhance the program across the Lifebridge Health system, ensuring all services align with our mission and SPIRIT values.

Key Responsibilities:

  • Program Development & Coordination:

    • Design, implement, and manage cancer survivorship and integrative medicine programs, ensuring comprehensive care for cancer survivors and their families.

    • Develop educational materials, including brochures, handouts, and online resources, to guide cancer survivors through their recovery and wellness journey.

    • Organize and lead educational workshops, support groups, and informational sessions to address survivorship concerns and wellness strategies.

  • Patient Navigation & Support:

    • Serve as the primary contact for cancer survivors seeking support, resources, and guidance related to survivorship and integrative therapies (e.g., acupuncture, nutrition, massage).

    • Provide education to cancer survivors on lifestyle changes, symptom management, and wellness practices to enhance post-treatment quality of life.

  • Collaboration & Integration:

    • Work closely with oncology and rehabilitation teams to ensure patients are aware of available survivorship resources.

    • Collaborate with multidisciplinary teams to integrate survivorship and wellness into ongoing care plans.

  • Data Collection & Program Evaluation:

    • Collect and track patient data to assess outcomes, program effectiveness, and satisfaction.

    • Monitor and evaluate the success of survivorship and integrative medicine programs, providing feedback and recommendations for improvement.

  • Continuous Improvement & Education:

    • Stay up-to-date with the latest research, trends, and best practices in cancer survivorship and integrative medicine.

    • Continuously assess and improve services to meet the evolving needs of cancer survivors.

  • Additional Duties:

    • Perform other duties as assigned to ensure program success and support for cancer survivors.

Qualifications:

  • Required:

    • Current Registered Nurse (RN) License in Maryland or eligibility to obtain Maryland license.

    • Oncology Certified Nurse (OCN) certification.

    • Basic Life Support (BLS) certification.

  • Preferred Skills:

    • Experience in cancer care, survivorship, or integrative medicine.

    • Strong communication, organizational, and patient-centered care skills.

    • Ability to work collaboratively with multidisciplinary healthcare teams.
