Job Summary: La Rabida is currently recruiting full-time RN Clinical Care Coordinators to join our outpatient unit in Chicago, IL. We are a small children's acute care hospital focused on treating complex medical conditions, chronic illnesses, developmental disabilities, and abuse and trauma. The CPNE is responsible for working with and providing clinical support for an identified group of patients and families and managing their ongoing health treatment plan. The CPNE actively supports the clinical care needs of the patients cared for by the physician they are aligned with within the outpatient clinic. The CPNE uses critical thinking skills and nursing expertise to ensure the timely completion of both clinical and episodic care coordination and ensures the successful attainment of HEDIS and other quality-related needs. Essential CPNE Functions: * Assists physician in all aspects of the consultation, treatment plan execution, and follow-up on the needs of the patient * Serves as clinical team liaison for families, care coordination entities, external agencies * Utilizes knowledge, skill, and expertise in pediatrics and/or a specialty to coordinate the clinical care of pediatric patients throughout the healthcare continuum * Responsible for the submission of prior authorization requests, while adhering to payer guidelines, for physician-ordered patient DME equipment, and medical supplies * Conducts review of and education with the patient/family as it relates to the physician-prescribed treatment plan What we need from you: * Graduate of an approved school of professional nursing. BSN required; MSN preferred * Three years of experience in pediatric nursing is required * Experience in care management or a clinical specialty is desirable * Current Illinois Professional Nursing Licensure; APN license if appropriate.
