SUMMARY:
Palliative Care Inpatient Nurse Navigator
The Palliative Care Inpatient Nurse Navigator provides specialized coordination and support for patients requiring palliative care services across the inpatient setting. This role ensures timely triage of consultation requests, efficient team assignment, and smooth transitions of care. The Nurse Navigator serves as an essential liaison between patients, families, providers, and multidisciplinary teams to promote high-quality, patient-centered palliative care.
RESPONSIBILITIES:
Consultation Management & Team Coordination
· Triage incoming palliative care consultation requests and assign them to the appropriate inpatient team.
· Review medical records and collaborate with referring providers to clarify clinical needs and recommendations.
· Follow established team assignment workflows and cadence as determined by Palliative Care leadership.
Patient & Family Support
· Provide education to patients and families regarding palliative care services, goals, and available resources.
· Distribute essential information to support informed decision-making and care planning.
Transition of Care
· Support transitions from inpatient to outpatient palliative care services, ensuring continuity and follow-through.
· Coordinate initial outpatient palliative care appointments and communicate with access associates for scheduling.
· Provide guidance and support for transitions of care to hospice when appropriate.
Interdisciplinary Collaboration
· Attend and participate in Multidisciplinary Rounds (MDR) with inpatient units to identify palliative care needs proactively.
· Serve as a liaison to the Emergency Department, supporting early identification and referral of patients requiring palliative consultation.
· Act as a Palliative Care Champion across hospital units, promoting awareness and integration of palliative care practices.
Education & Staff Support
· Provide education and support to clinical staff regarding palliative care service processes, best practices, and referral pathways.
Additional Expectations
· Demonstrate strong clinical judgment, communication skills, and patient advocacy.
· Maintain up-to-date knowledge of palliative care standards and institutional workflows.
· Support the ongoing improvement of palliative care service delivery through collaboration and feedback.
REQUIRED QUALIFICATIONS:
- Current licensure as an RN in the state of New York.
PREFERRED QUALIFICATIONS:
· Nursing experience in ICU, Oncology, Geriatrics, Emergency Department

PAY RANGE:
$80,753.00 - $110,292.00CITY:

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
