Terms and Conditions apply The Nurse Case Manager demonstrates a high level of expertise in the assessment, diagnosis, and treatment of the complex responses of individuals, families, or communities to actual or potential health problems; prevention of illness and injury; maintenance of wellness; and provision of comfort. Licensure, Certification, or Registration Requirements for Hire: Current RN licensure in Virginia or eligible or compact state Licensure, Certification, or Registration Requirements for continued employment: Current RN licensure in Virginia AHA BLS HCP Certification or equivalent Experience REQUIRED: Three (3) years of experience including two (2) in a specialty area Experience PREFERRED: Two (2) years case management/care management experience Education/training REQUIRED: Baccalaureate Degree in Nursing from an accredited School of Nursing Education/training PREFERRED: Certification in specialty area as appropriate Independent action(s) required: Makes appropriate decisions and initiates appropriate interventions in the provisions of care coordination within the boundaries of professional practice, the Standards of Practice for Case Management (CMSA) and established policies. Organizes and plans work with input from the client/family with specific outcomes and demonstrated use of sound clinical judgment and resources. Supervisory responsibilities (if applicable): N/A Additional position requirements: Determines the appropriate place of contact/intervention with customer including client's home, community agency, provider office, etc. in order to achieve customer satisfaction and a positive outcome.
