ACT Team Member functions as a member of the multidisciplinary team rather than an individual practitioner.  An ACT mental health professional assesses, plans, develops, coordinates, and directly provides flexible treatment, rehabilitation and support services to individuals who have the most serious symptoms of mental illness and who consequently have the greatest difficulty with basic daily activities such as keeping themselves safe, caring for their basic physical needs, maintaining a safe and affordable place to live, maintaining sobriety, and other activities of daily living.  Rather than brokering services, the team itself is the service delivery vehicle, and assumes direct responsibility for providing the specific mix of medically necessary services required by the persons-served, if there is a need. 

What You’ll Do

  • Promote a recovery-centered, person-centered environment and connect individuals with appropriate community resources.
  • Develop and update individualized treatment plans in collaboration with individuals served and the interdisciplinary team.
  • Coordinate and monitor services through regular face-to-face engagement and comprehensive tracking of activities.
  • Maintain accurate consumer records and documentation in compliance with company, CARF, and regulatory standards.
  • Provide crisis intervention and on-call support, including rotating nighttime, evening, and weekend coverage.
  • Provide substance use treatment and support services as appropriate.
  • Manage the medication system, including MAR oversight, medication storage, procurement, destruction, and administration/documentation of medications and injections.
  • Support safe and informed psychotropic medication management, including prior authorizations, refills, lab orders, and coordination of required blood work.
  • Conduct medical assessments and health screenings, including metabolic syndrome screenings, and promote wellness, prevention, and health education.
  • Use motivational interviewing and recovery-centered approaches to support medication adherence and help individuals develop health and wellness management skills.

What You Bring

  • Minimum – current license to practice as a registered nurse in Michigan.
  • One Year or more of experience in a clinical or rehabilitation setting with persons with severe mental illness
  • Maintain a valid motor vehicle operator license with driving records acceptable according to Hope Network policy.

  • Registered Nurse (RN) –ability to be on call 24/7 and take on call approximately 6 days per month
  • All ACT staff must obtain a basic knowledge of ACT programs and principles acquired through participation in MDHHS-approved ACT-specific initial training, and subsequent participation in at least one MDHHS-approved ACT-specific training annually thereafter. All initial training of ACT staff must occur within six months of hire for work in ACT.
