Admission/ Discharge/ Transfer, RN - Orthopedic Specialty Care Unit


What We Offer:

Location: Charlotte Orthopedic Hospital

Shift: 7A-7P, 3x12hrs. FT DAYS 

SOB: up to $15,000 

 

The Registered Nurse serves as a leader of the healthcare team at Novant Health by establishing an authentic personalized relationship with the patient and their chosen support system, collaborating with physicians and other team members to assess, plan, implement, and evaluate an individualized plan of care that promotes optimal health or supports a peaceful death. The Registered Nurse utilizes best scientific evidence and compassion to assist the patient in navigating their health journey. The Registered Nurse accepts responsibility, authority, and accountability for management and provision of care in accordance with the current policies and procedures.

What We're Looking For:
  • Education: 4 Year / Bachelors Degree, preferred.
    • Graduate of an accredited school of nursing required. If your RN nursing license has been inactive, retired or lapsed for five years or more and you have not been licensed in another state during the last five years, successful completion of a Board approved refresher course is required prior to reactivating or reinstating your license. In order to be considered for current licensure status, you must apply for reinstatement within one year of completing the refresher course.
  • Experience: One year relevant experience, preferred.
  • Licensure/Certification: Current RN licensure in appropriate state, required.
What You'll Do:

Role Purpose
Ensure safe, efficient, and timely patient admissions and discharges by completing assessments, education, and care coordination while supporting patient flow and unit operations.

 

Key Responsibilities

Admissions

  • Complete admission assessments, medication reconciliation, and documentation
  • Initiate care plans and safety screenings (falls, skin, nutrition, pain)
  • Verify orders and coordinate with providers and interdisciplinary teams
  • Orient patients and families to the unit and care expectations

Team Lead

  • Creating staffing assignments
  • Being aware of staffing needs
  • Escalating issues
  • Assisting nurses/ CNA’s with various tasks 

Discharges

  • Complete discharge education, medication reconciliation, and instructions
  • Coordinate follow-up appointments, services, and equipment needs
  • Ensure prescriptions, transportation, and documentation are finalized
  • Use teach-back to confirm patient understanding

Care Coordination

  • Collaborate with case management, pharmacy, therapy, and providers
  • Identify and escalate barriers to discharge in real time
  • Support throughput goals and reduce delays

Quality and Safety

  • Ensure compliance with regulatory, documentation, and safety standards
  • Monitor for high-risk patients and escalate concerns promptly
  • Participate in audits, process improvement, and unit initiatives

Pay
