Job Title: Nurse Team Leader – Home Care (Bronx, NY)

Locations: Bronx, NY (Field-Based Role)

Compensation:

• Salary Range: $125,000 - $135,000
  (Dependent on education, experience, nursing certifications, and bilingual skills in Spanish, Cantonese, or Mandarin)
• Sign-On Bonus or Loan Assistance: $15,000, paid over 3 years

Overview:
A leading home care agency in New York City, is seeking a Nurse Team Leader to join their exceptional team—based Bronx, NY
These positions are perfect for compassionate and experienced RNs looking to step into a leadership role. The Nurse Team Leaders will provide clinical oversight, coordinate care plans, and mentor field nurses, all while ensuring patients receive the highest standard of care.

Responsibilities Include:

• Managing and overseeing individualized care plans for patients.
• Supervising and mentoring home care field nurses, including onboarding new team members.
• Coordinating with interdisciplinary teams and families to ensure continuity of care.
• Documenting care activities effectively using EMR systems.
• Acting as a clinical leader in the field, offering guidance and support.

Key Qualifications:

• Current NY State RN License (required).
• BSN degree (preferred).
• At least 3 years of home care field experience (required); leadership or supervisory experience (a plus).
• Oasis Certification (preferred).
• Valid driver’s license (required for the Brooklyn-based role).
• Proficiency in EMR systems and Microsoft Word.
• Strong clinical knowledge of home health regulations and exceptional problem-solving skills.
• Bilingual fluency in Spanish, Cantonese, or Mandarin is a plus.
