Nurse Manager / CHHA – Home Care

Hybrid (2 Days in Office / 3 Days Remote)
Location: New York, NY 10041
Compensation: $135,000 – $150,000
Plus $15,000 Sign-On Bonus OR Student Loan Assistance (paid over 3 years)

RCM Health Care Services is partnering with a highly regarded Certified Home Health Agency (CHHA) in New York City to recruit an experienced Nurse Manager to lead and support a dynamic home care nursing team. If you’re an RN leader with deep home care experience and strong regulatory knowledge, this is an excellent opportunity to step into a high-impact management role.


About the Role

The Nurse Manager oversees the delivery of high-quality, compliant home care services by providing leadership, coaching, and operational support to nursing staff. The position manages RN Coordinators of Care, Community Health Nurses, and LPNs, ensuring clinical quality, regulatory compliance, and a strong patient experience.

This is an office-based role with a hybrid schedule:
2 days onsite Manhattan, NY
3 days remote


Compensation & Incentives

  • Salary: $135,000 – $150,000
  • $15,000 Sign-On Bonus

Key Responsibilities

Leadership & Staff Development

  • Mentor and support nursing staff to ensure high-quality, evidence-based home care services.

  • Partner with the Clinical Director to identify training needs and develop clinicians.

  • Conduct performance evaluations and ongoing supervisory visits.

  • Promote engagement, satisfaction, and retention across the nursing team.

Clinical Oversight & Quality Assurance

  • Assess clinician performance in documentation, coordinated care, and adherence to best practices.

  • Develop and monitor action plans to improve staff performance.

  • Oversee daily scheduling to ensure continuity, appropriate staffing, and productivity.

  • Provide clinical guidance to on-call staff after hours when needed.

Regulatory Compliance

  • Ensure adherence to federal and NYS DOH home health regulations, OASIS-E requirements, STAR ratings, and agency workflows.

  • Ensure timely, accurate processing of clinical orders.

  • Investigate complaints and incidents in accordance with agency guidelines.

Operational & Administrative Leadership

  • Collaborate with HR, Compliance, Finance, and other departments as needed.

  • Monitor productivity and participate in annual budget planning.

  • Support staffing needs and participate in the hiring and selection process.

  • Ensure appropriate use of clinical resources and supplies.


Qualifications

  • Bachelor’s degree required; BSN preferred

  • Master’s degree a plus

  • Minimum 3 years’ experience in a Certified Home Health Agency (CHHA)

  • Prior supervisory or management experience preferred

  • Strong knowledge of: OASIS-EDOH and federal home health regulationsClinical outcome measuresSTAR ratings

  • Excellent communication, critical thinking, and leadership skills

  • Bilingual (Spanish, Cantonese, or Mandarin) candidates are eligible for salary differentials
