Requirements & Qualifications

• Active Connecticut RN license
• Graduate of an accredited nursing program (BSN preferred)
• Minimum 1 year of hospice experience
• Valid driver's license and reliable transportation
• CPR Certification
• Strong assessment, communication, and case management skills
• Computer proficiency with electronic medical records

Must Haves

• Manage a hospice caseload from admission through end-of-life care
• Perform comprehensive hospice assessments and develop individualized care plans
• Coordinate care with physicians and the Hospice Interdisciplinary Group (IDG)
• Provide symptom management, pain control, and end-of-life nursing interventions
• Educate patients and families on disease progression, medications, and the dying process
• Coordinate services with facilities, caregivers, and community resources
• Complete timely hospice documentation and regulatory compliance requirements
• Participate in weekend/holiday on-call rotation as scheduled

Preferred Background & Skills

• Previous hospice or palliative care experience
• Home Health or Visiting Nurse experience
• Strong end-of-life symptom management skills
• Medicare Hospice Conditions of Participation knowledge
• Case management experience
• Experience working with interdisciplinary care teams
• Excellent patient and family education skills
