The role suits a clinically strong Registered Nurse with recent home health case management experience, strong assessment skills, solid documentation habits, and the ability to independently manage an assigned patient caseload in the field.

Schedule

Full-time, field-based, serving Fort Worth and surrounding Tarrant County.

Compensation

$85,000 to $92,000 annually, depending on experience.

What You'll Do

The Home Health RN Case Manager manages an assigned home health patient caseload in the field, performing skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges. This person develops, updates, and coordinates individualized plans of care in collaboration with physicians, patients, families, and the interdisciplinary care team, and completes accurate, timely clinical documentation including OASIS assessments and visit notes.

The role monitors patient progress, identifies changes in condition, and communicates updates appropriately to the care team, while providing patient and caregiver education on disease management, medications, safety, and plan-of-care goals. This person coordinates closely with therapy, social work, home health aides, physicians, and clinical leadership, and maintains compliance with home health regulatory, documentation, and quality standards throughout the patient caseload.

Required Qualifications

• Active Texas RN license in good standing
• Recent Home Health RN Case Management experience
• Strong understanding of Medicare home health regulations, OASIS, and plan-of-care management
• Ability to work independently in the field while maintaining strong communication with the clinical team
• Valid driver's license, reliable transportation, and ability to travel locally throughout Fort Worth and Tarrant County

Preferred Qualifications

• COS-C (Certified OASIS Specialist – Clinical) certification
• Experience with electronic medical record (EMR) systems
• Strong clinical judgment, organization, and time-management skills
