Someone with your depth of compassion, clinical insight and strong decision-making capabilities is the ideal person to map out and oversee the process of a patient's recovery from illness, injury or surgery at home. * You will provide a full range of advanced clinical home care services, including patient assessment, medication management and patient/family education * You will assume the ongoing, primary responsibility to coordinate, implement and continually evaluate the home care needs of your patients * You will review patient insurance information and coordinate activities involved in each individual care plan and make sure that pertinent findings are shared with family and professional caregivers Qualifications: * Current NYS RN license and registration required * Bachelor's degree in nursing preferred; Associate's degree required * Minimum of one year acute medical-surgical nursing experience required * Certified Home Health Agency (CHHA) experience preferred * Valid Driver License and Car * Bilingual English and Spanish or Mandarin or Cantonese preferred Fee for Service Visit Rates: Start of Care Weekend/Holiday Visit Rate: $120 (Maintain the premium for 3+) Start of Care Weekday Visit Rate: $100 (Maintain the premium for 3+) Revisit Rate: $85 Orientation Daily Rate: $250 In-Service Hourly Rate: $55 Apply
