University of Colorado Anschutz Medical Campus Department: Community Practice Job Title: RN Case Manager - Family Medicine, Louisville Position #00809017 - Requisition #32356 Job Summary: The RN Case Manager works collaboratively with physicians, interdisciplinary teams, patients, and families to promote positive patient outcomes. They perform a care continuum process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to support the individual's health needs. They utilize skilled communication, education, and resources to promote quality and cost-effective outcomes. The RN Case Manager provides focused support to various areas such as utilization management, value-based performance teams, emergency departments, and acute, ambulatory, and specialty care teams. Key Responsibilities: * Evaluates and identifies members' needs. Interfaces with Primary Care Providers, Specialists, and various disciplines on developing case management plans/programs. * Monitors and evaluates the effectiveness of the case management plans and modifies them as necessary. * Coordinates the interdisciplinary approach to providing continuity of care, including utilization management, value-based care and transfer coordination, discharge planning, and obtaining all authorizations/approvals/transfers for outside services for patients/families. * Acts as a clinical liaison, per their specialty, with outside agencies and facilities, outside providers, employers and/or workers' compensation carriers, and third-party administrators. * Prepares reports, communicates program changes to appropriate staff, and develops protocols according to state regulations. * Acts as a patient advocate and educator to assure that the patient has the knowledge to care for his/her condition and that the patient is educated and empowered to participate in the care plan. Qualifications: Minimum Qualifications: * Bachelor of Science in Nursing or a related field. * At least three (3) years of nursing experience. * Registered Nurse (RN), State of Colorado. Applicants must meet minimum qualifications at the time of hire. Preferred Qualifications: * 3 years job-related/clinic experience. * RN experience in Primary Care or Care Management setting. * Nursing certification (ANCC) in specialty area. * Bilingual, Spanish-speaking. * Computer skills: EPIC, Kronos. * MS Office: Outlook, PowerPoint, Word. Knowledge, Skills, and Abilities: * Ability to interpret, communicate and document with proper medical terminology. * Strong knowledge of CPT & ICD-10 procedure coding. * Strong knowledge of value-based care and risk-based coding procedures in Primary Care. * Ability to adhere to all policies and procedures, including but not limited to standards for safety, attendance, punctuality, and personal appearance. * Strong organizational and time-management skills. Anticipated Pay Range: The starting salary range (or hiring range) for this position has been established as $76,517 to $97,330. The above salary range (or hiring range) represents the University's good faith and reasonable estimate of the range of possible compensation at the time of posting.
