TRANSITIONAL CARE PROGRAM- Seeking RN for Full Time position. The 67 bed unit offers a state-of-the-art rehabilitation gym. Use your acute care skills on this sub-acute unit and be a part of an interdisciplinary team that assists patients in reaching their goals! Our Transitional Care Program provides post-acute care including: cardiac care, orthopedic care, stroke recovery care, pulmonary care, and medically complex care.
