Hospice RN Liaison Manager


# JOB DESCRIPTION SUMMARY: Under limited supervision, responsible for the management of patient care delivery and referral source relationships through oversight of Liaison RNs and Mobile Response Liaisons RNs as well as performing the duties of Liaison RN as necessary. Works with colleagues to ensure the achievement of Gilchrist#s marketing goals and objectives. Develops, implements, manages and markets the Gilchrist Mobile Response program in partner hospitals. Ensures assigned staff members establish and nurture external professional relationships, maintain awareness of local medical markets, provide clinical assessments of prospective patients when necessary, and represent Gilchrist and its spectrum of services in a competent, professional manner consistent with the organization#s goals. Manages the human, fiscal and communication resources to facilitate exceptional customer service to ensure the successful admission of our patients.# Education Registered Nurse or Bachelor/Master#s Degree in a related field; an equivalent combination of education and experience may be considered Experience 5-7 years# experience required in hospice, home health, critical care, ER, oncology or medical/surgical field Skills Ability to develop and successfully implement new service offerings Analytical skills necessary to organize and oversee the work of others and to administer sound policies, procedures, budgets and clinical practices Ability to market to and deal tactfully with customers (e.g. physicians, social workers, nurses, etc.), other possible referral sources, and the community Excellent professional communication, negotiation, presentation and public relations skills Self-directed with the ability to work with little supervision in a rapidly changing environment Flexibility in scheduling and ability to manage multiple tasks simultaneously Strong clinical assessment and documentation skills Strong computer skills including, e-mail, EMR, Word, Excel and PowerPoint applications Ability to mentor and develop staff Licensures, Certifications Valid driver#s license and safe driving record and an automobile that is insured in accordance with GHC requirements C.H.P.N. or Advanced Certified Hospice # Palliative Care Social Work certification required within three years of hire Physical Requirements Able to provide patient care that involves lifting, repositioning, moving patients and includes bending, reaching and twisting Ability to travel to patient locations; includes ability to drive, negotiate stairs and walkways, including uneven surfaces at private homes or facilities Sufficient dexterity to utilize electronic record keeping systems, computers/keyboards and or laptops and other related equipment Vision ability to assess patient#s health status using visual inspection and observation and to read medications, charts, calibration and monitoring devices Speech to verbally communicate to assess and impart information concerning patient status; to effectively complete patient/family teaching and to interact with internal and external health care teams Working Conditions Normal office environment with driving as large part of the day in the field, which includes visits to hospitals, skilled nursing # assisted living facilities, and patient#s homes, some in unsafe areas (escort provided) Exposed to adverse working conditions due to performance of patient care activities and exposure to communicable diseases Conditions of Employment # # JOB DESCRIPTION SUMMARY: Under limited supervision, responsible for the management of patient care delivery and referral source relationships through oversight of Liaison RNs and Mobile Response Liaisons RNs as well as performing the duties of Liaison RN as necessary.
