Home Care Transition Nurse

Overview The Home Care Transition Nurse is responsible for assessing, planning, and developing referrals. * 5 x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends * Great Healthcare benefits, and education reimbursement of $7,000 annually * Additional compensation for visits over your caseload Job Details Our Home Care Transition Nurses: * Establish and maintain professional relationships with key physician groups and referral sources. * Act as Liaison to referral sources on behalf of Catholic Home Care. * Identify potential market growth opportunities and program expansion. * Serve as a Customer service representative and act in a professional and supportive manner at all times. Requirements: Registered Nurse with current licensure in NYS * 2 or more years of home care experience preferred * Customer Service and Marketing skills required Salary Range USD $90,000.00 - USD $105,000.00 /Yr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
