Teaching Hospital Pharmacist (Evenings)

The Department of Pharmacy at SUNY Downstate Health Sciences University is seeking a full time Pharmacist (Evenings). The successful candidate will: * Dispense, compound, administer drugs and other pharmaceuticals * Provide medication counseling and reconciliation * Review medication orders for safety and efficacy and then approve orders within our CPOE system to main medication profiles * Review and approve Pyxis profile orders * Administer oncology, TPN, and investigational drug solutions as needed * Dispense controlled substances as needed and maintain files * Participate in drug utilization review studies, QA, and PI programs * Actively participates in new technologies and operational programs * Maintain the records of Pharmacy * Supervise technicians and support staff * Report to Senior pharmacists, the Associate Director of Pharmacy, and the Director of Pharmacy * Provide in-service education to healthcare professionals on demand * Potentially rotate with medical teams and/or participate in specific medication management programs * Holiday coverage rotation and On-Call/Recall rotation required Required Qualifications: * Doctor of Pharmacy (PharmD) from an accredited College of Pharmacy * New York State Pharmacist Licensure Preferred Qualifications: * Prior hospital pharmacy experience * PGY-1 completed residency Work Schedule: Variable Days; 3:30pm to 12:00am (Evenings) Salary Grade/Rank: SL-3 Salary Range: Commensurate with experience and qualifications Executive Order: Pursuant to Executive Order 161, no State entity, as defined by the Executive Order, is permitted to ask, or mandate, in any form, that an applicant for employment provide his or her current compensation, or any prior compensation history, until such time as the applicant is extended a conditional offer of employment with compensation.
