Role: The RN, Care Coordinator is responsible for assessing and identifying patient/family needs, utilizing the nursing process, coordinating the Plan of Care with the Interdisciplinary Group (IDG), and providing palliative and supportive care to the patient/family unit. Educational/Training Requirements Associate's degree in nursing required; BSN preferred Certificates/Licenses/Registration Graduate of an accredited RN program Florida Registered Nursing license CHPN Certification desired Valid Florida Driver's License with good record Automobile Insurance Clear Level 2 Background Check Some locations may require: * Provides reassurance on the phone to patients and families. Assists in finding solutions to their questions and/or recognizes the need for an in person visit. Coordinates in person visit when needed/or requested. * Assists the patient and family to achieve the desired comfort levels. * Utilizes appropriate support/expert resources or personnel to resolve complex or difficult situations. * Documents patient/family contact information in the EMR and communicates with the Interdisciplinary Group (IDG). Experience Requirements Minimum one year nursing experience strongly desired Position Specific Responsibilities (not all inclusive) To provide high quality individualized nursing services to patients. Provide direct and indirect nursing care to assigned patients. Work under the direct and indirect supervision of the Care Manager or designee but is independent in the application of advanced nursing knowledge and skills. Assessment: Collect patient data and complete required forms with appropriate responses according to unit standards. Identify patient's overt problems/needs and set priorities. Identify problems requiring further referral and/or follow up. Interpret and record latest diagnostic results. Perform advanced nursing assessment using critical thinking skills. Planning Care: Develop a plan of care based on nursing process which incorporates the plans of other disciplines and continuing care needs. Include the patient/family in developing or revising the plan of care. Make referrals to interdisciplinary support services. Implementing/Managing Care: Care provided conforms to accepted practice standards. Provide correct treatments/procedures and other care as prescribed and according to patient care standards. Demonstrate understanding of age-related characteristics and needs of patients served. Explain nursing procedures in appropriate forms. Provide discharge teaching. Identify emergency situations and independently initiate advanced therapy. Act as coordinator of patient care with other health care personnel and evaluate patient care measures instituted. Understand and demonstrate respect for patient rights and utilize established mechanisms for management of ethical issues in patient care. Evaluation: Record patient care delivered as planned and any variation with appropriate rationale. Make and record observations related to impending or associated problems. Leadership: Using nursing process, delegate nursing care to appropriate personnel. Integrate cost effective measures into nursing practice. Recognize unit problems and take responsibility for documenting same. Demonstrate active participation in QI processes. Comply with hospital expectations to meet staffing demands based on patient care needs. Comply with policies addressing safe working conditions. Monitor unsafe working conditions. Recognize inappropriate and/or ineffective patient care management. Resolve issue/problem and complete written reports. Fulfill call for unit as assigned and adjust staffing for census (volume) and acuity. Quality/Compliance: Adhere to all aspects of organizational compliance and regulatory issues. Incorporate quality into all aspects of responsibilities as evidenced by audits. Audits will demonstrate assessment, planning, implementation, patient and family education, evaluation and documentation. Represent the company in a positive and professional manner at all times with others, e.g., co-workers, management, vendors, and customers. Consistently represents Hope with integrity and professionalism. Complete all job responsibilities in a thorough and complete manner, utilizing good judgment in all aspects of the job. Carry out duties and responsibilities in a manner that exemplifies excellent customer service. When unsure of a directed task or any part of job responsibilities, proactively seek the assistance of immediate supervisor. Assist with special projects, as requested. All other job duties as may be assigned. Pay

Average Florida Staff Position Pay

$47.37/hour

The average salary for a Med Surg is 5% higher than the US average of $45.
