Nurse Practitioner, Telehealth

For more information, visit www.oakstreethealth.com. Role Description OSH is building an exciting and innovative new program to take the best care of our patients possible. We want to meet our patients where they are at and we know many of our patients want to stay in the safety of their own home. We're looking for NPs to complete important annual assessments for our patients via Telemedicine (in particular, via best-in-class video technology). This role will be fully remote and the work hours are 10am to 7pm CST. We partner with a network of elite specialists and hospitals for specialty and acute care. This role is vital for OSH to deliver best-in-class primary care, care coordination, and population health. Responsibilities include: * Provision of exceptional primary care. * Annual wellness visits and health risk assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific documentation. * 100% of visits via Video * Care coordination with other providers, specialists, testing facilities, agencies (both within OSH and third party providers) * Population health leadership, in coordination with the Care Team * Patient education. * Other duties as assigned This role reports to the Director of Population Health and works closely with the Oak Street Health Physicians Group, P.C. leadership. What we're looking for * Nurse Practitioners with experience in primary care: internal medicine, geriatrics, or family medicine. * Experience with risk adjustments and/or annual assessments * Understands HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) Coding, and Health Risk Assessments (HRAs). * Active, non-probationary state license. * Genuine passion for primary care. (Done well, the practice of medicine should be fun.) * Intrinsically motivated and prefer work hard/play hard. * Embrace teamwork and the opportunity to collaborate with brilliant colleagues. * Want to be a part of an innovative model focused on empirically-guided population health. * Spanish language skills strongly preferred, depending on clinic location. * Open to working 10am - 7pm CT or 10am - 7pm ET.
