Nurse, Clinical Service Liaison

Job Details: Responsibilities * Patient Clinical Educator/Business Development (clinical - traditional/specialty) * RN (Registered Nurse) or LPN (Licensed Practical Nurse) * Current licensure as a Registered Nurse in the state(s) where the Home Infusion Center conducts business. Additional licensure may be required in multi-state service area * Provide bed-side/virtual clinical education to both the patient and caregiver to include hospital hook ups when indicated. * Full Time (this is not a work from home position); F2F daily with customers; goal is to see F2F customers each day to work on building the relationships, being available to provide patient/caregiver/prescriber educational interaction related to patients transition from hospital/clinic/prescriber office to home. * Marketing specialty/traditional infusion capabilities to message every facet related to home infusion education/messaging on behalf of the health systems home infusion services. * Collaborate, Coordinate and Communicate with CarepathRX intake to provide details needed related to a new patient referral and the patients seamless transition to home infusion services. * Quarterback the patient referral process. * Responsible for branding/marketing to all potential specialty home infusion customers. * Strong communication skills, specialty therapy marketing experience, ability to multi-task, detail multiple specialty therapies and disease state focus planning. * Qualify referral source opportunities * Payer knowledge and ability to navigate Medicare, Medicaid and Commercial insurance plans to assist with the intake benefit and authorization process. * Strategic planning experience and territory optimization execution. * Conducts in-services introducing services, therapies, drug detailing, and products to referral sources. * Follow up with all new referrals and patient care communications with the referral sources, physicians, and internal and external teams. * Participates in all activities related to customer pull-through, process improvement and ongoing communications related to managing and growing the business. * Consistent communication and relationship building of Home Health partners to streamline the discharge process of each new patient transitioning to the home. * Hold an active, unencumbered nursing license by the state Board of Nursing or Nursing Regulatory Body * Hold and maintain current BLS certification Physical Demands * Required to drive.
