Non-acute care is offered through various affiliates, Summary: This role is significant to the success of the RNs on the Telemetry Unit and, as a result, patient experience, satisfaction and hospital outcomes. Serves as Patient navigator on the unit; work with people, their families, and their caregivers to overcome barriers to care while in the hospital Supports the RNs, to ensure cardiac monitors are available and in good working order; reviewing, prioritizing and responding to patients; organizing documents; maintaining records; and any other administrative tasks that help the RNs, PCTs, UCs, MT and patients. A prime importance to this position is the ability to support and maintain control, order under stressful conditions during periods of high unit activity, while utilizing effective and respectful communication with patients, families, and the health care team. Responsibilities: * Ensures critical equipment is available and in working order on the unit. When necessary, performs basic troubleshooting on telemetry equipment; liaises with BioMed to on more complex repairs. * Transfer and confirm telemetry boxes between CMU and patient rooms. * Manage/Allocate resources. * Patient Navigator - provide guidance to patients, assist with barriers that prevent them from getting the care that they need. * Coordinate, delegate and prioritize tasks to maintain the daily flow of the unit in a complex, rapidly changing environment. * Communicate with staff, families, and other departments utilizing effective communication skills. * Effectively coordinate communications involving incoming calls, pages, and nurse call light system * Coordinate admissions, discharges, and transfers of patients; maintains medical records and release of information for continuation of care. Ensures timely room turnover and patient experience to maximize reimbursement for care provided. • Effectively manage various administrative tasks such as entering patient charges, ordering supplies, and other duties as assigned. * Proactively identifies issues and takes actions to avoid negative impact to patient care. * Able to appropriately process the flow and events occurring within the unit to maximize patient experience which positively impacts reimbursement. Other information: Minimum High School Diploma Preferred Work Experience Minimum three (3) year of experience in a medical or customer service environment or experience of a similar nature Preferred three (3) year of experience in a medical environment. Minimum Preferred Required Skills, Knowledge, and Abilities * Demonstrate effective and respectful communication with patients, families and the healthcare team, using excellent interpersonal skills * Ability to prioritize tasks, multi-task and manage interruptions quickly and appropriately in a rapidly changing environment. * Complete work timely and accurately. • Manage multiple stakeholders, inside and outside of the department. * Work effectively and collaboratively within a team environment. * Demonstrate exemplary telephone etiquette. * Demonstrate a high level of attention to detail which is critical when dealing with patient care.
