Responsibilities * Hypertrophic Cardiomyopathy/Heart Failure RN The section of Cardiology has an exciting opportunity for an RN Navigator / Educator who wants to be part of a growing Hypertrophic Cardiomyopathy/Heart Failure Program. Our position will give the unique opportunity to both share in the coordination/growth of a division wide (Hypertrophic Cardiomyopathy) HCM program while also being involved in patient care and education for our heart failure patients. We are looking for someone with a passion for program development and strong clinical and professional skills. Key components of this position are listed below: HCM Navigator Component (50%) * Develop a rapidly growing program providing comprehensive multidisciplinary care for patients with hypertrophic cardiomyopathy, with responsibilities ranging from patient care to developing outreach programs and education. * Provide care coordination for patients with HCM undergoing a multitude of comprehensive diagnostic and therapeutic interventions, including alcohol septal ablation, surgical septal myectomy, genetic testing and advanced cardiac imaging. * Organize and facilitate bi-weekly multidisciplinary HCM conferences attended by cardiologists, radiologists, cardiac surgeons and anesthesiologists across the VMFH system as well as help plan the inaugural HCM CME day. * Enroll and monitor patients on new HCM drug Mavacamten as well as helping with in-clinic genetic testing for hypertrophic cardiomyopathy. Heart Failure Educator Component (50%) * Collaborate with a multi-disciplinary team consisting of heart failure and transplant trained cardiologists, cardiac surgeons, interventional cardiologists, advanced practice providers, social workers, and pharmacists to coordinate, support, and optimize the medical care of this complex patient population. * Provide care for individual patients over the phone using standardized HF protocols. In addition to clinic and phone triage, the HF RN Educator is responsible for patient education regarding medications, life-style interventions, and dietary recommendation. * Integrate patient and family education into the delivery of care, ensuring educational activities are culturally appropriate and are tailored to the patient's needs. Coordinates the Heart Failure patient experience throughout the Virginia Mason Franciscan Health System. The position will focus on HCM program growth and coordination in the mornings and will support patient education/care for our heart failure patients in the afternoon on a 5-8's schedule. Qualifications * Current Washington State licensure as an RN * CPR certification is required every two years * Strong patient assessment skills, ability to work collaboratively in team setting, current working knowledge of electronic medical records.
