SUMMARY OF POSITION: This APP will work in General Pediatrics to provide care to children from birth through young adulthood with in-depth knowledge and experience in pediatric primary health care including well-child care and prevention/management of common pediatric acute illnesses and chronic conditions. They will also work in Complex Care, a section within General Pediatrics. The complex care program is responsible for the overall medical management of children with medical complexity: patients who have some type of medical technology (i.e. tracheostomy, gastrostomy tube), are followed by multiple subspecialists and require frequent emergency room visits and hospitalizations. The nurse practitioner/physician assistant will be responsible for inpatient consults when patients of complex care are admitted to the hospital to provide continuity of care, outpatient hospital follow-up visits, acute sick visits, as well as answering phone calls and messages from families and other agencies involved in the child's care. The nurse practitioner/physician assistant will also assess complex care program referrals to determine eligibility and schedule where appropriate. The complex care team as a whole is designed to provide a medical home for these fragile children and attempt to decrease emergency room visits as well as length of stay when hospitalized. MINIMUM QUALIFICATION(S): * Masters Degree in Nursing with clinical specialization required. * Two (2) years' work experience as a RN required. * Current licensed to practice as a CRNP by Pennsylvania Board of Nurse Examiners required. PREFERRED QUALIFICATION(S): * Certification as an NP in the area of practice specialization is preferred.
