Behavioral Health Care Manager (RN)


JOB SUMMARY The Behavioral Health Care Manager provides clinical case management services to clients receiving outpatient behavioral health services at WhidbeyHealth.# This position reports directly to the Behavioral Health Manager. PRINCIPLE FUNCTIONS includes the following, other duties may be assigned: Coordinates with all health services, and other internal and external service providers regarding clinical care, service delivery, treatment planning, discharge planning, and barriers to care. Consults with care team and/or BH manager on difficult to engage clients. Performs screenings and intakes within one week of the receipt of referral. Helps identify and address related problems affecting health of client, such as housing, vocational rehabilitation, and social integration, and coordinates service delivery with other health center staff and community providers.# Meets regularly with patients, engaging at least 5 patients per day to develop case plans and appropriate interventions. Monitors client progress through the course of treatment, re-evaluating and adapting the case plan at required intervals. Assists the Behavioral Health Clinicians and coordinates with primary care, including referring patients to community resources. Accompanies patients to appointments when necessary and makes home visits when appropriate.# # # # # # ## Provides case management support to the Medication Assisted Treatment (MAT) Program and behavioral health program which includes but not limited to identifying and addressing psychosocial concerns, engagement in services, coordinating care, advocacy, and health related barriers.### # # # ## Coordinates with all health services, and other internal and external service providers regarding clinical care, service delivery, treatment planning, discharge planning, and barriers to care. Consults with care team and/or BH manager on difficult to engage clients. Documents all patient encounters, including face to face, telephone, and electronic communications, in the medical record. Follows appropriate engagement and/or discontinuation of case management procedures. The BHCM work#closely with the care team to navigate the individualized care plan. May contact providers, programs, and or agencies to whom the patient has been referred to verify adherence to the plan. Facilitates multidisciplinary treatment planning meetings. Acts as an interdisciplinary team member within the Behavioral Health Department. Complies with all documentation requirements. Participates in the delivery of regulatory forms to patients when appropriate. Actively participates in clinical performance improvement activities. Supports the vision, mission, and values of the organization in all respects. Supports the principles of continuous improvement with energy and enthusiasm, functioning as a champion of change. Provides and maintains a safe environment for caregivers, patients, and guests. Conducts all activities with the highest standards of professionalism and confidentiality. Complies with all applicable laws, regulations, policies, and procedures, supporting the organization#s corporate integrity efforts by acting in an ethical and appropriate manner, reporting known or suspected violation of applicable rules, and cooperating fully with all organizational investigations and proceedings. Experience in case management preferred. Must be able to work as an independent problem solver.
