Nurse Practitioner

Are you an experienced post-acute care nurse practitioner (NP) who aims to make a difference in the lives of your patients by providing quality care? TeamHealth needs someone like you! We are seeking a part-time nurse practitioner (NP) to work in a skilled nursing facility environment rounding on patients 3 to 4 day a week. Key Responsibilities: * Perform primary care in a skilled nursing facility setting for short term and long-term patients * Manage chronic disease, diagnose illness and medical conditions, and implement plan of care * Order and interpret lab results * Prescribe and monitor medications * Consult with physicians and coordinate patient care with other healthcare providers * Educate patients and their caregivers on coping skills to handle chronic and age-related conditions * Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery * Promote healthy lifestyle changes Desired Attributes: * Critical thinking skills * Excellent communication skills * Ability to collaborate with the healthcare team * Strong clinical skills, work ethic, and ability to work independently * Adaptability and flexibility to address the fluctuations that occur during the aging process and with chronic illness * Empathy, compassion and ability to connect and engage with your patients.
