Nurse Monitor - Non-Merit - HHS Community Health Services


Regular Schedule: 17 to 34 hours per week. Several non-merit vacancy exists in the Community Health Services/Community First Choice Program of Baltimore County Department of Health and Human Services. Non-Merit positions are not classified within the Baltimore County Government Classification and Compensation Plan. Definition of Responsibilities Nurse Monitors provide quality assurance oversight (QA) ofResidential Service Agencies (RSAs) serving at-risk Medicaid participants. Nurse monitors liaison with Adult Evaluation and Referral (AERS) staff, Support Planners (SPs), and the Office of Health Care Quality (OHCQ) to facilitate the health and safety of participants in a variety of community settings. Monitors work flexible hours including evenings and weekends to meet the needs of participants who receive in-home services around the clock. Monitors possess medical-surgical/geriatric and/or pediatric experience, strong assessment skills, the ability to work independently in a home setting, and the capacity to instruct and evaluate providers of in-home assistance to vulnerable Medicaid participants across the age spectrum as needed. Monitors must be familiar with/able to learn and interpret applicable COMAR regulations. Monitors possess a high degree of electronic medical record experience/computer literacy and excellent documentation skills. This job announcement may close at any time with little or no notice. Examples of Duties Job Purpose Nurse Monitors provide quality assurance oversight (QA) ofResidential Service Agencies (RSAs) to ensure the health and safety of at-risk Medicaid participants. Essential Job Duties * Review State Tracking System (LTSS) data/interRAI assessments and any attachments upon assignment of cases. * Contact assigned clients/representatives timely. * Make home visits at a frequency determined by the medical/social-emotional status of the participant. * Conduct in person Quality Assurance reviews annually and as needed. Contact SPs and RSAs to coordinate care/resolve care issues promptly. * Verify presence and appropriateness of (nursing) care plans for participants. * Report deficiencies via LTSS to MHD and other agencies as needed. Examples of Other Duties Complete all documentation/submit all activity billing via LTSS timely and in keeping with Program policies/processes. Make joint home visits with SPs and other agencies (i.e. Adult or Child Protective Services) as necessary. Performs other related duties as required. Qualifications Required Qualifications This position requires a registered nurse with a Maryland license and a minimum of 2 years professional nursing experience. Education: Must possess at least an AA in nursing, bachelors in nursing or other field preferred. Experience:Must have at least 2 years medical/surgical experience. Licenses and Certificates Possession of a valid license to practice as a professional registered nurse, issued by the Maryland State Board of Examiners of Nurses, is required at the time of appointment. Possession of a valid driver's license equivalent to a noncommercial, class C Maryland driver's license is required at the time of appointment. All licenses and certifications must be maintained at current levels throughout employment. Proof ofLicenses, Certifications and Education Applicants are required to submit proof of licenses, certifications and education beyond high school to meet the required and preferredqualifications of the position. Conditions of Employment Nurse Monitors must be available to make visits/contacts outside regular County hours (8:30 am - 4:30 pm) and/or on weekends as needed. Monitors must agree to document all billable activities in the LTSS and submit activity billing timely. Licenses and Certificates Possession of a valid license to practice as a professional registered nurse, issued by the Maryland State Board of Examiners of Nurses, is required at the time of appointment. Possession of a valid driver's license equivalent to a noncommercial, class C Maryland driver's license is required at the time of appointment. All licenses and certifications must be maintained at current levels throughout employment. Physical and Environmental Conditions The work of this classification may include the performance of physical activities such as walking, standing, and lifting, the operation of medical equipment and instruments, and travel throughout the county. Duties may expose incumbents to various bio-medical hazards. Medical Examination and Employment Background Investigation Applicants selected for employment with Baltimore County must successfully complete a physical examination, drug screening and employment investigation, including, but not limited to criminal background, education certification and fingerprinting checks.
