This is accomplished by providing coordination between outpatient and inpatient care management teams, physicians, advanced practice providers and post-acute care. Skilled in developing collaborative relationships with internal and external stakeholders. * Ability to use independent clinical judgment and compassion when carrying out tasks. * Excellent organizational skills. * Computer skills with proficiency in Microsoft Office & Outlook.
