JOB SUMMARY # The RN is an integral member of the Cardiac and Pulmonary Rehab clinic and works The RN is responsible to the day-to-day planning and conducting of specific exercise and lifestyle management assessment and intervention for individuals participating in the program. #The RN works collaboratively within the multidisciplinary team to provide care to patients who have entered the clinic#s program. # PRINCIPLE FUNCTIONS includes the following, other duties may be assigned: # Conducts phase II Cardiac Rehabilitation and Pulmonary Rehabilitation according to hospital and department policies and procedures. Assesses the comprehensive nursing needs of an assigned group of patients. Continuously plans, implements, and evaluates patient care, incorporating patient and family/significant other into the process Plans, conducts, and demonstrates specific exercise and lifestyle assessment and interventions that meet the needs of each individual patient including nutrition, wight intervention, lipid management, stress management, and smoking cessation. Designs and implements individualized goal-oriented exercise prescription for cardiac and pulmonary rehabilitation clients. demonstrates and supervises the use of rehab equipment to patients and new staff (treadmill, stationary bicycle, wall pulleys, free weights, rowing machine, and arm ergometer, etc.). Conducts new patient interviews which include but not limited to medical history, functional status, short and long-term goals, nutritional assessment, and emotional status. Provides patient education as it applies to cardiovascular disease risk factor modification. Designs individualized treatment plans for clients entering cardiac and pulmonary rehab and provides 30-day updates to the referring physician and/ or cardiac rehab Medical Director. Must be able to set up, monitor, and read EKG/ECG systems used during cardiac rehabilitation and exercise. Ensures that patients and their support systems understand education provided as to the patients# medical condition, plan of care, and established exercise goals. Maintains exercise equipment with regards to cleaning and inspection. Maintains that crash cart and defibrillator is in working order. RN appropriately reports defibrillator malfunction in timely manner to bioengineer and subsequently arranges and carries out replacement plan. Understands procedure for oxygen delivery to designated patients. Coordinates with RT and EP in O2 tank replacement procedure.# Coordinates with RT and EP to check and maintain that O2 tanks always contain 1000 PSI Completes all required feedback and communication to patient and healthcare team including EMR documentation and reports concerning education, exercise progression, exercise treatment, and monitored physiological and vital responses of the patient. Identifies own learning needs and makes plans to meet these needs. Actively participates in and supports departmental and organizational quality initiatives and goals. JOB KNOWLEDGE # QUALIFICATIONS Education RN degree from accredited school of nursing.# Current WA RN licensure.# # ############################################### ############## # # # # # # # # # # # # # # # Training and Experience Competent in telemetry and possesses current BLS certification.# ACLS certification is required within one month of employment. One year experience in providing health care services for cardiac patients with ECG monitored patients preferred.## Experience in Outpatient Cardiopulmonary Rehab setting with telemetry monitoring. Experience with cardiac monitoring, rhythm, blood pressure and oximetry interpretation and assessment required.###################################################################### ######## Understanding of hemodynamics and pathophysiology of cardiopulmonary diseases.# Ability to communicate effectively and establish rapport.
