What you'll do The Nurse Supervisor - Dialysis and Home Dialysis will work Full-time, Days, and will be a healthcare professional who supervises and oversees a team of dialysis nurses in various settings, including the Cooperstown Inpatient Dialysis Hospital Unit, Outpatient Dialysis Center, and Network Home Dialysis. The primary responsibilities of the Nurse Supervisor - Dialysis and Home Dialysis include managing and directing the day-to-day activities of the nursing teams, ensuring patient care is delivered in accordance with established protocols and procedures, and maintaining a safe and efficient work environment. The Nurse Supervisor - Dialysis and Home Dialysis is also responsible for training and mentoring new staff, evaluating staff performance, and developing and implementing quality improvement initiatives for each area. In addition to supervisor duties, the Nurse Supervisor - Dialysis and Home Dialysis is responsible for providing direct patient care, including administering dialysis treatments, monitoring patient vital signs, and providing education and support. The Nurse Supervisor - Dialysis and Home Dialysis is responsible for all aspects of home dialysis including, education, patient training, clinic visits, approving invoices, and maintaining an on-call schedule during non-operational hours providing direct patient care to the home and hospitalized patients as needed. The home dialysis supervisor will assure that Nursing Tiers are covered to teach inpatient floor staff to do CAPD exchanges as well as the peritonitis protocol. The Nurse Supervisor - Dialysis and Home Dialysis collaborates with other healthcare professionals, such as the Director of Dialysis, Technical Supervisor, Nephrologists, Social Worker, and Dietitians to ensure that patients receive comprehensive care. The Dialysis Nurse Supervisor must be knowledgeable in regards to the dialysis water system and regulatory requirements related to water treatment in the dialysis setting, ensuring compliance with relevant guidelines. This position requires home visits. * The Nurse Supervisor - Dialysis and Home Dialysis provides management and supervision of the Inpatient, Outpatient, and Home Dialysis Units. * Consistently provides staff supervision which focuses on professional growth and development and desired patient outcomes as monitored by staff feedback and monitoring by supervisor * The Nurse Supervisor - Dialysis and Home Dialysis maintains clinical staff competencies for Inpatient, Outpatient, and Home Dialysis, offering an annual validation program * Assures staff compliances with job description and healthcare requirements. * Consistently performs staff performance appraisals, discipline, realistic goal setting and remediation as monitored by staff feedback and supervisor review * The Nurse Supervisor - Dialysis and Home Dialysis consistently provides a staffing schedule, which meets required patient: staff ratios and takes into account staff requests as monitored by staff feedback and supervisor review * Consistently supports staff education needs by scheduling in-service presentations. Facilitates staff attendance at BLS, ACLS and other educational opportunities as staffing and funds allow as monitored by staff feedback and supervisor review. Assures staff compliance to job description and healthcare requirements. * The Nurse Supervisor - Dialysis and Home Dialysis consistently collaborates with patients, families and providers in creating a patient schedule which maximizes patient productivity within the framework of available staff and operating hours of the facility, as monitored by patient, family and provider feedback * Consistently coordinates and supervises patient training and education by colleagues as monitored by patient feedback, staff feedback, and supervisor review. * The Nurse Supervisor - Dialysis and Home Dialysis consistently coordinates and publishes monthly on-call schedules for both home dialysis and inpatient dialysis * Regularly holds staff communication meetings, keeps minutes, follows up and facilitates solutions as monitored by staff feedback and supervisor review * Provides nursing care, training, education, and home dialysis support to adult ESRD patients and significant others involved in their care at home. * Trains and educates patients and significant others utilizing up to date information as monitored by patient feedback and supervisor review. * The Nurse Supervisor - Dialysis and Home Dialysis consistently assesses the need for home visits and follows up appropriately as monitored by patient feedback and supervisor review * Consistently provides support for home patients and family members while maintaining professional boundaries * The Nurse Supervisor - Dialysis and Home Dialysis consistently utilizes the nursing process when assessing patients and creating individualized plan of care. * Provides direct patient care as needed in response to on-call issues. * Collaborates with the MD and staff on all hospitalized home patients providing direct patient care as needed * Resources to nursing staff * The Nurse Supervisor - Dialysis and Home Dialysis participates in program development. * Consistently serves as an active member of the department Performance Improvement committee as monitored by supervisor * The Nurse Supervisor - Dialysis and Home Dialysis is responsible for QAPI tracking and action plans to meet goals. * Is responsible for Core Team meetings * Assures Care plans are up to date and complete. * Meets with Medical Director monthly to drive QAPI goals. * Consistently participates in coordinator meetings, demonstrating the ability to recommend change and propose solutions through a collaborative effort with colleagues as monitored by feedback from other coordinators and supervisor review * The Nurse Supervisor - Dialysis and Home Dialysis consistently demonstrates initiative and implements approved change projects as monitored by feedback from other coordinators and supervisor review * Consistently contributes to the education of colleagues as monitored by staff feedback and supervisor monitoring * Supports the dialysis teaching day and assists with the planning * Provides direct care to adult ESRD patients * Functions as clinical expert and role model 100% of the time when working in patient care as monitored by staff feedback and supervisor review * The Nurse Supervisor - Dialysis and Home Dialysis consistently leads interdisciplinary efforts to improve patient outcomes as monitored by feedback from staff, physicians and supervisor * Regulatory Compliance * Assures100% compliance with Crown web reporting * Assists with data entry into Crown Web * Runs monthly reports to support QAPI activity * Sustains survey readiness and all regulatory requirements * The Nurse Supervisor - Dialysis and Home Dialysis maintains up to date policies and procedures * Performs similar or related duties as requested or directed * Performs other duties as requested and observed by supervisor or manager What we need from you Education: * 2 Year / Associate Degree required * Graduate from an Baccalaureate, Associate or Diploma school of nursing required, BSN desired All RN's hired and do not currently hold a BSN will be expected to demonstrate enrollment in a bachelor's prepared nursing program within 4 years of hire. BSN must be completed within 10 years. RN's licensed on or before 12/19/17 are/remain exempt from this requirement. This position is eligible for a sign on bonus up to: USD $35,000.00 Pay Transparency Salary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards. Pay Range Minimum: USD $86,097.70/Yr. Pay Range Maximum: USD $129,146.55/Yr.
