Summary of Position This job description provides information essential to understanding the scope of the position and is not intended to be an exhaustive list of skills, efforts, duties, responsibilities or working conditions associated with the position. The Nurse Navigator functions on the multidisciplinary team as an advocate, interpreter, educator, and counselor for oncology patients. She/he is responsible for ensuring all adult patients with an oncology diagnosis receive quality and comprehensive services based on best practice and national guidelines. He/she will coordinate patient care throughout the continuum from diagnosis through survivorship in collaboration with the multidisciplinary team. She/he will serve as a clinical resource with expertise in hematology/oncology care management.# He/she will serve as a liaison throughout the facility and in the community regarding services provided for this unique patient population. He/she will provide expert nursing care, which includes: direct clinical practice, consultation, education and research. # PRIMARY DUTIES # RESPONSIBILITIES (ESSENTIAL): These requirements are representative, but not all-inclusive, of the knowledge, skill, and ability required of the position. Direct Clinical Care (direct patient and family education, navigation # support) Provide support for the emotional and educational needs of the patient (family) newly diagnosed with cancer. (Ex. Formal and Informal Pre-treatment/procedure education, introduction to cancer center)########################## Patient advocate and navigator to facilitate transitions through phases of care by providing support and education, utilizing best practice and national guidelines, in collaboration with all members of the team. (Ex. Active participant in interdisciplinary rounds; Care Coordination of identified patient population)###### Quality Participates in the development and implementation of CI activities for the OESS program to constantly improve processes Collaborates with members of the interdisciplinary team throughout the Oncology Service Line to ensure consistency and quality of service delivery across settings. Actively participates in Cancer Committee and the achievement of standards to support the Commission on Cancer accreditation process. Professional Development Maintains knowledge and skills through self-development and participation in continuing education opportunities on a regular basis to expand knowledge base and role expertise. Additional Responsibilities Provides back-up nursing support throughout the program depending on expertise. (Ex.
