This unit cares for patients who are above 17 years of age who require medical/surgical nursing care. The primary population served is surgical, including but not limited to, orthopedics, bariatrics, vascular, neurology and general surgery. # This unit also serves general medical patients with a diagnosis including but not limited to: sepsis, respiratory diseases, gastrointestinal diseases, skin and wound conditions and infections. Modified primary nursing is the care delivery model utilized and includes the following team members: Registered Nurses, Certified Nursing Assistants, Case Managers, Discharge Planners, Social Workers and Unit Secretary. The team is also supported by a unit-based Pharmacist, a Respiratory Therapist, a Physical Therapist, and a Unit-based Educator. This position is responsible for assessing patient/family health problems and resources and taking a leadership role in the development, implementation and outcomes evaluation of a nursing care plan provided by the nursing team. Provides direct nursing care and interventions to designated patient populations. This#role requires providing services to one or more age populations, including adolescents through geriatric, in a manner that demonstrates an understanding of the functional/developmental age of the individual served. This position requires full understanding and active participation in fulfilling the Mission of Blessing Hospital.
