Recognized as one of the best places to live in Maryland, Frederick is surrounded by mountain views and centrally located about 45 miles north of Washington D.C. and 45 miles west of Baltimore, MD.   JOB SUMMARY: The primary responsibility of the Registered Nurse is to provide primary health care services to clients who experience homelessness, are uninsured, whose primary language is one other than English, and/or who are identified as low-income. Duties include working with a team of health care professionals to engage clients in services by participating in outreach and intake efforts, performing nursing assessments and health screenings, conducting health education sessions, making referrals, and assisting in the day-to-day operations of the health care center. This individual will receive direct supervision from the Supervisor of Community Health Center. Occasional medical on-call responsibilities may be required. As a Federally Qualified Health Care Center (FQHC), The City's Housing & Human Services employees may qualify for a loan repayment program.  These programs are offered through the National Health Service Corps (NHSC) and are available to those providing primary medical, dental, or behavioral health care services or evidence-based substance use disorder.     ESSENTIAL DUTIES AND RESPONSIBILITIES: * Provides direct client services such as nursing assessments, health screenings, intakes and medical histories, health education, first aid, administer medication, change wound dressings, and medical case management services. * Works with a collaborative team to research ways to improve healthcare processes and improve client outcomes.  * Collaborates with physicians and licensed nurse practitioners in determining the treatment regime most beneficial for client care. * Triages clients via telehealth, phone and in-person for acute and chronic concerns. * Collects patient health history and performs diagnostic testing. * Supports daily operations of the health center by supporting scheduling efforts and ordering supplies. * In partnership with Supervisor of Community Health Center, may oversee day to day activities performed by other nurses and auxiliary personnel in health center, * Supports clients and family members with discharge instructions and treatment planning. * Provides referrals to medical, mental health, and addiction treatment programs. * Provides off-site nursing services to persons who are homeless or are unwilling or unable to access onsite health care services. * Maintains a cooperative relationship among health care teams by communicating information and participating in team problem-solving methods. * Maintains all paper and electronic records as needed; performs data entry tasks as assigned. * Responds to environmental accidents and assists in the general maintenance and cleaning of the agency especially the health care clinic. * Supports team by assisting with other casework, client services, and clinical duties as needed.  * Performs other duties as assigned.  QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.   REQUIRED EDUCATION AND/OR EXPERIENCE: * Bachelor's degree in Nursing from an accredited college or university. * Two (2) years of experience as a Registered Nurse in a community health center.  REQUIRED CERTIFICATES AND/OR LICENSES:  * Current licensure to practice as a Registered Nurse in the State of Maryland. Employee must maintain the license for the duration of their employment. * Vehicle Operator's license with satisfactory driving record that meets insurability standards of the City's insurance carrier is required. The employee must maintain the license for the duration of their employment. * Basic Life Support certification. Employee must maintain the license for the duration of their employment. SKILLS (Communication/Language; Mathematical; Computer; etc. * Ability to communicate using the Spanish language. * Knowledge of holistic service delivery models. * Good clinical skills in areas of patient assessment and history taking. * Ability to multitask, supervise and make long-range plans for community health services.  * Strong interpersonal communication skills and an ability to provide services to persons who are low-income, have limited English proficiency and/or who experience homelessness. * Good written and oral communication skills. * Ability to use Microsoft Office Suite. OTHER DESIRABLES: * Ability to communicate using American Sign language. * Background in psychiatric and/or behavioral health nursing.
